This service is by appointment only.
Please note
You will need to bring all appropriate documents with you when you attend your appointment. These will usually be; proof of address, proof of identification, proof of any benefits you may be applying under such as your Personal Independence Payment scores or the Disability Living Allowance mobility rate.
