Plan for emergencies and changes
Why planning for emergencies matters
There may be times when you are unable to provide care, for example if you become unwell or need to go into hospital.
Making an emergency plan can help other people understand the support the person you care for needs and who to contact if you are unable to provide care.
What to include in your emergency plan
Talk about the plan with the person you care for, and with family or friends who may need to help.
Write down:
- the name and address of the person you care for
- who should be contacted in an emergency
- important medication information and how to access medicines
- details of any ongoing treatment or care
- important daily routines
- details of professionals or services involved in their care
Keep the plan somewhere it can be easily found if it is needed.
