Call Staffordshire Cares on 0300 111 8010 to request the assessment. A relative or representative can contact us for you; we may also need to speak to you directly.

You can complete your financial assessment online through the Social Care Portal, or we can provide forms by post, email or telephone if this is more suitable for you.

What happens next:

  • We’ll send you the financial assessment form.
  • We can help you complete it if you need support or reasonable adjustments (phone appointment, interpreter or easy read).
  • If your savings are falling, contact us when they are around £30,000 so we can plan ahead.
