Two ways to share your experience
1. Quality experience forms – general feedback
Use these forms to share your experience of a care service. They help us understand overall quality and identify trends.
- questions are optional – answer only what you want.
- forms include ratings and space for comments.
- your details will stay confidential unless you agree to be contacted.
Quality experience form (easy read)
After you submit a quality experience form, we review the information to understand what works well and where improvements may be needed. Your feedback helps us spot trends across services rather than look into individual cases. We use what you share to inform our quality monitoring and, where appropriate, feed this into wider service improvements.
2. Quality assurance form – compliments or concerns
Use this form if you have a specific compliment or a quality concern about a care service.
- if you ask us to, we will share your concern with the service and request an investigation.
- if you share a compliment, we will make sure the service knows about it.
Use this form if:
- you have a particular issue that you couldn’t resolve with the service.
- you want to share a resolved issue for information purposes.
- you want to record a compliment.
After you submit a quality assurance form, we check it to make sure we have all the information needed to process it. If you’ve asked us to contact the service, we’ll share your concern with them and request an investigation. When we receive their response, we review it carefully to make sure it is detailed and appropriate. If you’ve shared a compliment, we record it and pass it on to the service.
What these forms are NOT for
- Safeguarding concerns: If you think someone is being abused or is at risk, call 0345 604 2719 or use the safeguarding referral process.
- Formal complaints: If you want to make a formal complaint, contact the service first. If you’re not satisfied, contact the Staffordshire County Council Complaints Team.
Why your feedback matters
Your views help us understand what’s working well and where we need to improve. We want to hear from:
- people who use adult social care services
- family members and carers
- care providers and professionals
- members of the public
You can share compliments, concerns, or general feedback about any adult social care service in Staffordshire. This includes services outside Staffordshire if the person using them is a Staffordshire resident.
Your privacy
If you give us your name and contact details, we’ll keep them within Staffordshire County Council. We might get in touch if we need more information about your feedback.
