If you are struggling with day-to-day activities because of illness, disability, age, or changes in your circumstances, we may be able to help.

You can complete an online self-assessment to tell us about your situation and the support you may need.

If you need help completing a self-assessment, contact Staffordshire Cares, we can support you through the process.

You may not need a social care assessment to get help

Many people can get the help they need without having a social care assessment. If you are looking for practical support, information, advice or help for yourself or someone you care for, the following services may help.

You can complete a self-assessment online or you can contact Staffordshire Cares to start discussing your care and support needs.

This helps us understand:

  • what support you may need
  • what difficulties you are experiencing
  • what services or information might help

The self-assessment usually takes around 15–20 minutes to complete, depending on your circumstances.

Need help or not sure where to start?

If you are not sure what support you need, would prefer to speak to someone, or need help completing the self-assessment, contact Staffordshire Cares and we can talk through your options.

Carers assessment

If you care for somebody who could not manage without your support, you may be entitled to support in your own right through a carer's assessment.

What information you'll need

When completing your self-assessment, it helps to have information about:

  • your contact details
  • your daily living needs
  • any difficulties with:
    • washing
    • dressing
    • eating
    • moving around your home
    • getting out and about
    • work or education
  • any support you already receive
  • equipment, technology, community support or help from family and friends
  • any health and safety concerns at home

If somebody else is helping you complete your assessment, they can provide information on your behalf.

What happens next

Once we receive your self-assessment:

  1. We will review the information you have provided.
  2. We may contact you if we need more information.
  3. We will decide whether a full assessment is needed.
  4. We will discuss the next steps with you.

How we decide if you are eligible for support

We follow the Care Act 2014 to decide whether you have eligible needs.

If you are eligible, we may offer short-term support, information and advice, or longer-term support depending on your needs.

If you are not eligible for adult social care services, we will provide information and advice about other support available in your community.

Support available after an assessment

If you are eligible for support, we will work with you to understand your needs and agree how best to meet them.

This may include:

  • information and advice
  • support planning
  • care and support services
  • equipment or adaptations
  • community-based support
  • short-term support to help maintain independence

If you are not eligible for support

Not everyone who contacts us will be eligible for ongoing adult social care services.

If you are not eligible, we will still provide information and advice about other ways to get help and stay independent. This may include community support, advice services, equipment, technology, support for carers and other preventative services.
