Application forms for admission to school

In-year applications

The following form can be completed if you wish to apply for a school place during the school year.

Please note: This must be returned to your chosen school in the first instance, who will respond to you directly. You should receive a written response to your application in 7 to 10 days.

An admissions application form for September 2021 entry will be available when applications open on 1 November 2020.

Supplementary Forms

This supplementary form can be emailed or posted with your completed application to the School Admissions and Transport Service or forwarded to the school directly. This is not an exhaustive list and you are strongly advised to check your preferred school's admission arrangements to see if a supplementary form is required.

Nursery Schools

Staffordshire County Council no longer administers the application process for Nursery. For information on how to apply, please contact your preferred setting directly.
