Please complete this form below if you are registering to work on school transport contracts. It should be filled in by the person named on the DBS application, and not by the company.
Notes before completing the form:
These roles are exempt from the Rehabilitation of Offenders Act 1974 and you are required to disclose any convictions, cautions, reprimands or final warnings which would not be filtered in line with current guidance which can be found on the government website.
Failure to disclose information which is later provided through the DBS process may result in the offer of this role being reconsidered.
Important – For New School Transport badge / Replacement School Transport badge (where DBS Update has expired) applicants
Before You Apply for a DBS
Applicants must complete the Self Disclosure Form and submit their SCC School Transport Badge application before applying for an Enhanced DBS certificate.
Enhanced DBS certificates issued before the SCC School Transport Badge application date will not be accepted.
Before applying for an Enhanced DBS certificate, applicants must:
- Complete a Self-Disclosure Form.
- Submit their SCC School Transport Badge Application and payment.
- Only then apply for an Enhanced DBS certificate.
