You can use this form to report missed care (home care, day service, transport) that you have received an invoice for, please note that if you have informed us of any cancelled or missed care, we will use this information to see if any changes need to be made to your next invoice, as this does not always result in a reduction of cost.
If you believe a person is at risk of abuse, neglect, unsafe care or serious harm, do not use this form alone. Report safeguarding concerns to Staffordshire Cares as soon as possible so appropriate support and action can be considered.
