- What you’ve tried: Equipment, technology, community support, or help from family/friends.
- Health and safety: Any risks, falls, or concerns at home.
How and when we’ll contact you
We aim to start your assessment within 28 days, although it may take longer, and we will keep you updated. Assessments can take place by phone, video, or through a home visit, and we will agree on the option that works best for you. You are welcome to have a family member, friend, or carer with you, and if you need help to understand the process, please let us know.
How we make decisions
We follow the Care Act 2014 to check whether your needs are eligible for support. If you are eligible, we may offer short‑term reablement to help you become more independent. If you are not eligible, we will provide information and advice about other types of support available.
Tip: Think about a “good day” and a “bad day,” and how long tasks take, whether they cause pain, distress, or are unsafe without help.
