Claim form

The paper form should be used for any child who meets the additional non-economic criteria below.

A child could be eligible if they:

  • have been looked after by the local authority for at least one day
  • have been adopted from care
  • have left care through special guardianship; and
  • are subject to a child arrangement order

This form must legally be completed / signed each term by the person with parental responsibility/legal guardians of children attending the setting and claiming Early Education Funding (EEF). There is a section that is relevant for Early Years Pupil Premium that should also be completed:
