Home care and reablement services

Question:
Home care and reablement services
Answer:

Independence at home serviceIndependence at Home logo

Independence At Home prevents hospital admissions and supports people to age in place and stay at home for as long as possible. We provide short-term homecare and reablement support to adults in Staffordshire, with a focus on maximising independence and wellbeing.

Our reablement service can be for up to six weeks and is designed to get you back on your feet and help you maintain your independence at home. We work with occupational therapy teams to identify aids and equipment that can help you.

Our homecare service is short-term care to ensure your needs are met as per your care and support plan. During this time, we will look for reablement opportunities to maximise independence and reduce dependence on formal care provision.

Referral into the Independence at Home service is via the Home First and relevant social work teams.

The service is provided by Staffordshire County Council and is regulated by the Care Quality Commission.

The Independence at Home service is based at:

Castle House
Barracks Road
Newcastle-Under-Lyme
