Make a request for information
The relevant forms are provided here for you to make requests for information from Staffordshire County Council. Please read the information below carefully to ensure that you are making the right type of request and so that you avoid any unnecessary charges.
Individuals have the right to make a request for a copy of their personal information. Alternatively, an individual appointed to act on their behalf can make a request for them.
Adult Social Care Files
To access a copy of your records, you should see your social worker or address your request to your local area office. You will need to include the relevant identity documents and a fee up to the statutory £10 we may charge. You will contacted if you are required to pay a fee upon receipt of your request.

Children's Records
If you wish to access a copy of your records when your case is open or close, you should contact the Children's Information Officer|. Information will be provided free of charge. You may be asked to collect the records in person.

Education Records
Requests for education related files should be made through the Information Governance Unit. You will need to include the relevant identity documents and statutory £10 fee prior to release.
