Survey of Voluntary and Community Sector Support Needs within Staffordshire and Stoke-on-Trent

This survey is for voluntary and community organisations working in Staffordshire and Stoke on Trent. Your views are very important and will be used to inform future service planning to ensure that voluntary and community sector infrastructure support commissioned locally by the public sector meets the needs of its users.

Please try to answer all the questions. Please read through the whole questionnaire and all the options before making your choice. Although the survey looks quite long, nearly all the questions are select boxes and it should only take about 15 minutes to complete.

A summary of the key findings from this consultation will be available on the Staffordshire Strategic Partnership website in due course. Please view this at: http://www.staffordshirepartnership.org.uk/thirdsector

Your individual response will be confidential to Staffordshire County Council and will not be shared with any third parties. If you have any questions please contact Nicola Edmonds, Third Sector Relations Manager on 01785 854920, or Denise Smallman, Head of Third Sector Commissioning Partnership Programme on 01785 276452.

All completed surveys must be returned to us by no later than Sunday 20th November 2011.  If you are returning a hard copy of this survey, please use the prepaid envelope provided.
