Archive Service Events Booking Forms

Please download the booking form for the event or course that you wish to attend.  The address to which the booking form should be returned (together with your payment) will be specified on the form.

Most of our events have limited availability and tickets are allocated on a first come, first served basis.  If your booking form arrives after all available tickets have been allocated your cheque will be returned to you.

Please note that refunds cannot be made unless a cancellation is received 8 working days in advance of the event.

The booking forms are provided in PDF format, the standard format for public service documents.  Adobe Reader software is required in order to read them.
