The Social Care and Health Directorate
Local Services within Eight Districts
Services are now delivered within eight localities, coterminous with the borough/district council areas of:
Staffordshire Moorlands, Newcastle, Stafford, Cannock, Lichfield, East Staffordshire, Tamworth, South Staffordshire.
each District is structured as per the diagram below.

The simple management structure is designed to be:-
- Capable of providing joint leadership for a range of public service functions.
- Adaptable and flexible over time as services change, develop and integrate.
- Accessible and comprehensible both to customers and staff.
- Easy to hold to account.
Our services are now configured to ensure we can deliver our changing lives agenda:
Access Team
The purpose of Access is to deliver a time limited, front line service to all adults 18 and above that is accessible across the whole client range as appropriate. It requires a strong commitment to improve the customer journey to offer an equitable, flexible approach to meet the needs of the person and carer preferably within their own home ensuring a whole systems approach is required.
Access is focussed on three defined areas, to provide and effective, efficient, flexible, integrated approach - Care support and information, Prevention and promoting independence and Signposting/care pathways.
Enablement Teams
Enablement picks up ongoing casework (including hospital discharge work) that extends beyond the seven day remit of the access team and works across all vulnerable groups - this team has a key role in respect of crisis intervention. Enablement services will ensure people regain and retain maximum independence in the shortest possible time. This service will be available for adults; older people and people with disabilities.
Well Being Teams (Older People/Disabilities)
More complex assessments, DFG, adaptation work will be managed here as will reviews, carers assessments etc. People requiring continuing professional support with long term conditions would also be supported by Well Being. The sensory impairment agenda will be managed here, as will transition form children's to adults services.
