Living Independently Staffordshire

Living Independently Staffordshire is a brand new service that has been launched.

It's an important new service that will give people more choice and more support to live independently following illness or injury, whether leaving hospital, or simply starting on the road to recovery.

We know that more people want to live at home and until now it has not always been possible, but this new service can provide up to 12 weeks of free intensive support if needed, which is based on an assessment. It will help you to get back the independence you deserve.  We can help you to regain skillsman in wheelchairor develop new ways of doing things to that you are more in control.

So what is different?

Previously people could have up to 6 weeks free reablement care.  But we found that this wasn't always enough time to help people get back on their feet and sometimes people would end up back in hospital again or in long term care.  So we are increasing the support to up to 12 weeks which we know will help more people become independent again and not end needing so many services.

The other great thing about this service is that it will be available to people over 18 if they are referred by their GP, hospital or direct from the community.  No other county in the country offers reablement care to people as young as 18.

The service will be more flexible and offers more choice to make sure that you get the support you need. 

Through the pilot schemes we have tried and tested the new approach and more than 50% of people who used it were able to go back to their own lives and live independently without further support. 

It is great news for residents of Staffordshire that while other counties are cutting social care and in particular reablement services we have been able to invest in ours.  

What support can I expect? 

Support will range from simple help with medication and relearning how to boil a kettle, to man in wheelchair and family in parkmore intensive support like regaining the ability to talk and carry out other normal daily tasks.

It's all about building personal confidence through a flexible approach that gives people more choice and more control over their care. 

Each package of care will be worked out with the support of the Community Reablement  Co-Ordinator to make sure it is individually tailored for you.
