Transition Checklist

This series of booklets has been developed by parents and carers of children with special needs/disabilities, who are part of SUN (Staffordshire Umbrella Network) South Staffs.

The "Transition Checklist" booklets have been written, following a transition event held in 2009, to tell other parents "what we wish we had known when our child was going through transition".

Information in the booklets is given in good faith and every effort has been made to ensure accuracy as at January 2011. However, no responsibility is accepted for the accuracy of information, nor does inclusion in this booklet constitute a recommendation.

Parents have been supported by Staffordshire Parent Partnership Service in producing these booklets. Printing has been funding through Staffordshire Aiming High for Disabled Children.
