In the South Staffordshire (Wombourne, Codsall and Perton) network group, parents and carers felt that they would like to look at the transition into adult life for young people with additional needs.
The Transition Checklist booklets have been written following a transition event held in 2009, to tell other parents "what we wish we had known when our child was going through transition".
Information in the booklets is given in good faith and every effort has been made to ensure accuracy as at February 2012. However, no responsibility is accepted for the accuracy of information, nor does inclusion in this booklet constitute a recommendation.
Parents have been supported by Staffordshire Parent Partnership Service in producing these booklets. Copies of the booklets can be made available on request, in large print, Braille, on audio tape and in languages other than English, please contact Parent Partnership Service for more details 01785 356921.
